What Is ICL Surgery?

ICL surgery places a soft, flexible STAAR lens behind your iris and in front of your natural lens. It can correct short-sight and, for selected patients, long-sight without removing corneal tissue or your natural lens.

Blue Fin Vision® uses EVO or EVO+ ICL for short-sight and hyperopic Visian ICL for long-sight where appropriate. Toric lenses are available when astigmatism also needs correction.

The procedure takes around 20–30 minutes per eye under local anaesthetic. You normally return home the same day.

An ICL can be removed or exchanged if your needs change. This still involves surgery inside the eye, so it is not a consequence-free undo.

Every ICL patient at Blue Fin Vision® is included in our Lifetime Monitoring Protocol™. This provides scheduled checks of the lens position and vault, the space between the ICL and your natural lens, alongside monitoring of the cells that help keep your cornea clear.

Is ICL Right for You?

ICL may suit you if you have:

Short-Sight

ICL can be considered for selected short-sighted prescriptions from around −0.50 D to −18.00 D. This includes higher myopia and patients whose thin or irregular corneas, dry eyes or prescriptions make laser a less suitable option.

Long-Sight

Hyperopic Visian ICL may be considered for selected long-sighted prescriptions up to around +10.00 D. Your eye needs sufficient space in front of the natural lens, and the natural lens must be clear.

For some patients, particularly when reading vision is changing with age, lens replacement may be a better option. Your consultant will discuss this as part of the same assessment.

A Preference for ICL Over Laser

You do not have to be unsuitable for laser to consider ICL. You may have a stable, lower short-sighted prescription but prefer an implantable lens that does not remove corneal tissue.

Contact-lens intolerance, dry-eye concerns, sport or a preference for a removable implant may all influence your decision. England footballer Lucy Bronze chose EVO ICL at around −3.00 D despite being suitable for laser eye surgery.

Every patient follows the same assessment pathway. You only proceed if ICL is the right option for your eye, prescription and goals.

What Happens Next

In-person assessment required before surgery: If you wish to proceed, we arrange the detailed assessment needed to confirm suitability and plan surgery.

Your clinical assessment includes biometry, detailed imaging, lens sizing and a prediction of the vault between the implant and your natural lens.

If your lens or eye anatomy requires a laser peripheral iridotomy, known as a PI, this is usually performed with a YAG laser at Harley Street one to two weeks before implantation. It is included in the treatment fee.

Routine PI is generally unnecessary for myopic EVO ICL because the lens has a central port. PI is often required for hyperopic Visian ICL. Your consultant will explain whether it is needed for your eye.

After implantation, you return home the same day. You attend scheduled follow-up appointments, followed by monitoring through the Lifetime Monitoring Protocol™.

Precise ICL Sizing

An ICL sits behind your iris and in front of your natural lens. Its size and position matter: a lens that sits too close to the natural lens or too far forward may cause problems.

At Blue Fin Vision®, ICL planning uses:

Swept-source OCT imaging: Anterion and Revo FC provide detailed images and measurements of the front of your eye.
Sulcus-to-sulcus measurement: This helps your consultant assess the space where the ICL will sit.
Vault prediction: This helps guide lens selection before surgery.
Biometry and eye-health checks: These assess the space inside your eye, your natural lens and the cells that help keep your cornea clear.

The Blue Fin Vision® target vault range is 250 to 750 microns. Vault measurements are available at Blue Fin Vision® sites. Endothelial cell counts are performed at Harley Street according to the Lifetime Monitoring Protocol™ schedule.

What to Expect

Before Surgery

Your consultant confirms your eye measurements and ICL size, explains the procedure and discusses the risks and expected recovery with you. You complete formal consent before treatment.

If a PI is required, it is arranged at Harley Street before your implant procedure and included in the treatment fee.

During Surgery

The procedure takes around 20–30 minutes per eye under local anaesthetic drops. Through a small self-sealing incision, your surgeon inserts and positions the ICL behind your iris. No stitches are required.

After Surgery

You rest in a private, en-suite recovery room before going home the same day. We provide:

Vision often improves soon after surgery, although recovery varies from person to person.

ICL Surgery Cost

From £3,750 per eye. The ICL costs page lists treatment for both eyes from £7,500. Your quote will show the lens and treatment recommended for you. If a toric lens changes your price, this will be made clear before you proceed.

Risks of ICL Surgery

All eye surgery carries risk. You may not achieve the result you hoped for and, rarely, surgery inside the eye can cause serious loss of vision.

Risks include a rise in eye pressure, cataract developing later, a vault that is too high or too low, loss of endothelial cells that help keep the cornea clear, and a remaining prescription that needs further treatment. Some patients require a PI before implantation. An ICL may need repositioning, exchange or removal.

Your consultant will explain the risks relevant to your eyes before you decide whether to proceed.
