The Implantable Collamer Lens (ICL) is an excellent long-term refractive solution. Unlike laser vision correction, which reshapes corneal tissue, the ICL is a device implanted inside the eye. Two clinical parameters, endothelial cell density and ICL vault, must be monitored for the life of the implant. Both can change silently. Neither is detectable by patients. Both are measurable. Both have defined clinical thresholds at which intervention is considered.
At Blue Fin Vision®, monitoring is not a goodwill gesture. It is a clinical protocol: structured, scheduled, objective, threshold-driven, and documented in the electronic patient record.
The Protocol
The Blue Fin Vision® ICL Lifetime Monitoring Protocol™ is provided to every patient who undergoes ICL surgery at Blue Fin Vision®. It is included in the cost of surgery. There is no separate enrolment fee, no subscription, no card to carry, and no benefit that expires.
The Protocol delivers structured objective monitoring, the scans, imaging, and reports, for the life of the implant. Consultant consultation is triggered by the Protocol only when a monitoring finding crosses a defined clinical threshold or flags a clinically significant change.

The Lifetime Measurement Model™
The Protocol is an application of the Blue Fin Vision® Lifetime Measurement Model™, the clinical philosophy that every implanted device is the start of a measurement relationship, not the completion of a procedure.
Under the Model, what is measured, at what intervals, against what thresholds, and what is done about the results is published, not promised. The Model is the umbrella principle from which every Blue Fin Vision® monitoring protocol is derived, and it is a clinical extension of the Blue Fin Vision® Clinical Proof System™.
Monitoring Across the Blue Fin Vision® Network
Monitoring visits are available across the Blue Fin Vision® network, with the Harley Street flagship clinic supported by centres at Weymouth Street, Chase Lodge Hospital in North West London, Phoenix Hospital Chelmsford in Essex, and One Hatfield Hospital in Hertfordshire. Vault measurements are performed at every site; specular microscopy for endothelial cell counts is performed at Harley Street. This gives patients greater geographic access to Harley Street standards of monitoring, rather than a franchised alternative.

What Is Monitored
Three parameters are tracked for the life of the implant:
- Endothelial Cell Count (ECC). Measured by specular microscopy. Assessed for absolute cell density and for longitudinal rate of loss against an age-adjusted baseline.
- ICL Vault. Measured by anterior segment OCT (AS-OCT) and Pentacam Scheimpflug imaging. Assessed against the published safe range and against each patient’s own baseline.
- Anterior Segment Health. Slit-lamp biomicroscopy, intraocular pressure, and crystalline lens assessment, with formal review for early lenticular change.
Monitoring Schedule
Every patient follows a structured schedule. Patients with accelerated cell loss, borderline vault, or any anterior segment concern are moved onto an accelerated pathway at the operating consultant’s discretion.

Post-operative consultant reviews at Months 1, 3, and 12 are included within the ICL surgery fee, not within the Protocol. The Protocol covers scheduled imaging and monitoring from Month 12 onwards, for life.
What Is Included
At no additional cost, for the life of the implant:
- All scheduled monitoring visits
- All imaging: specular microscopy, AS-OCT, Pentacam Scheimpflug tomography
- IOP, slit lamp, and refraction at every monitoring visit
- A written monitoring report after each visit, documenting measurements against the patient’s own baseline
- Structured, timestamped entry into the Blue Fin Vision® Electronic Patient Record
- Year-on-year longitudinal comparison reporting
- Consultant consultation, triggered by a monitoring finding. If any measurement crosses a defined threshold or a clinically significant change is identified, a consultant-led review is arranged at no additional cost as part of the Protocol.

What Is Not Included
The Protocol covers objective monitoring and consultant review triggered by monitoring findings. The following fall outside its scope:
- Routine consultations in the absence of a monitoring-detected clinical trigger. Standard Blue Fin Vision® consultation pricing applies.
- Self-requested second opinions and symptom-led consultations unrelated to monitoring findings. Standard Blue Fin Vision® consultation pricing applies. Patients with acute or urgent concerns are prioritised clinically, irrespective of billing.
- Management of unrelated ocular pathology, including glaucoma, macular disease, diabetic eye disease, and corneal disease not attributable to the ICL
- Refractive error unrelated to the original ICL indication
- Contact lens fitting or trial
- Cosmetic lid or ocular surface procedures
- Review of ICLs implanted elsewhere: non-Blue Fin Vision® ICL patients may enrol via a separate paid assessment pathway
- Travel, access, and accommodation costs
Defined Clinical Intervention Thresholds
Intervention is considered, not automatically triggered, when any of the following are observed, and is interpreted in full clinical context by the operating consultant:
- Vault. Outside the published safe range of 250-750 µm, sustained across two successive measurements, or drifting significantly from each patient’s baseline.
- Endothelial Cell Count. Absolute density trending below 2,000 cells/mm², or a rate of loss exceeding the age-adjusted expected trajectory.
- Crystalline lens change. Any new cortical or anterior subcapsular opacity, or refractive drift consistent with early lenticular change.
Decisions are made by the operating consultant with the full longitudinal dataset visible on a single timeline. Possible actions include accelerated monitoring, ICL exchange, ICL explantation, or transition to the Post-ICL Cataract Surgery Pathway.
Post-ICL Cataract Surgery Pathway
Every ICL patient will, in time, develop a cataract. The ICL does not cause cataract, age does, but the anterior segment anatomy is altered by the presence of the implant, and the surgery is more complex than a standard phacoemulsification. Blue Fin Vision® is the only UK ophthalmic group to define this pathway in advance and publish it with transparent pricing.
The Pathway includes: ICL explantation, phacoemulsification, and a standard monofocal or toric intraocular lens, with the full Blue Fin Vision® perioperative and follow-up package.

What the Pathway Includes
- Pre-operative consultation, biometry, and surgical planning

What the Pathway Does Not Include
- Premium presbyopia-correcting intraocular lenses (trifocal, EDOF), available as an optional upgrade at Blue Fin Vision®’s transparent differential pricing
- Surgery for coexisting ocular pathology unrelated to the ICL or cataract
- Non-standard surgical complexity arising from trauma or unrelated disease
Patients whose monitoring visit identifies a developing cataract, or who notice cataract-type symptoms between scheduled visits, are moved into this Pathway directly. No external referral is required.
Why Monitoring Is a Protocol, Not a Promise
A guarantee depends on the provider continuing to exist. A protocol is documented, transferable, and anchored in clinical governance: it survives the passage of time. An ICL is an implant for decades. Monitoring for decades requires a protocol, not a promise.
Every measurement, every visit, every threshold breach is recorded in the Blue Fin Vision® Electronic Patient Record. The dataset belongs to the patient and is available on request.
Enrolment
Every ICL patient operated on at Blue Fin Vision® is automatically enrolled in the Protocol at the point of surgery. There is no separate consent form, no subscription, and no opt-in. The Protocol is a clinical commitment, recorded in the electronic patient record alongside the surgical note.
Clinical Takeaway
An implanted lens is not a finished procedure. It is the beginning of a measurement relationship that runs for the life of the implant. We measure what matters, at defined intervals, for as long as the lens is in the eye, and we price the consequences in advance.
