What Is a Chalazion?
A chalazion, also called a meibomian cyst, is a rounded lump in the eyelid. It is not an infection, and many resolve without treatment given enough time.
Small chalazia often cause no symptoms beyond their appearance. Larger ones can press on the eye and blur or distort vision, rub against the eye, or make the eyelid feel heavy.
Blue Fin Vision® offers consultant-delivered chalazion removal across London, Hertfordshire and Essex. Your named consultant performs every assessment and procedure personally, explaining whether further home treatment or removal is appropriate. We treat patients aged 16 and over.
A stye is different: it is a painful, red infection at the eyelid margin that usually settles within a week or two. A stye can leave a chalazion behind once the infection clears.
What Causes a Chalazion?
A chalazion forms when one of the meibomian glands, the oil-producing glands inside the eyelid, becomes blocked. Trapped oil leaks into the surrounding tissue and triggers a slow inflammatory reaction, producing the lump.
Chalazia are more common in people with:
- Blepharitis, inflammation around the eyelid margins
- Meibomian gland dysfunction, where the eyelid’s oil glands do not work properly
- Rosacea, a skin condition that can also affect the eyelids
- A history of previous chalazia
Warm compresses, gentle lid massage and lid hygiene are the first steps in managing a chalazion and help support the health of the eyelid glands.
Is Chalazion Removal Right for You?
Chalazion removal may be suitable if you are aged 16 or over and have:
- A lump that has lasted more than four to six weeks despite regular warm compresses
- A chalazion that is large or growing
- Pressure on the eye or blurred vision
- Chalazia that keep returning
- A persistent lump that concerns you enough that waiting longer does not feel reasonable
Some features suggest that an eyelid lump may not be a chalazion. These need prompt assessment:
- Eyelash loss over the lump
- Ulceration or bleeding
- Thickening of the eyelid margin
- A lump that returns in exactly the same place
A lump with these features is not managed as a routine chalazion. Book an assessment rather than waiting for it to settle.
Your consultant will confirm the diagnosis and explain whether removal or further conservative treatment is appropriate.
What Happens Next
- Book your consultation: Arrange an appointment online. Consultations are available every day across our network.
- Consultant assessment: Your consultant examines the lump, eyelid margin and eye to confirm whether it is a typical chalazion.
- Personalised recommendation: We explain your options, including waiting. If treatment is appropriate, it can often take place at the same visit.
- Treatment and aftercare: You return home the same day with chloramphenicol eye ointment and written aftercare instructions.
Chalazion Removal: Treatment Options
Your consultant will recommend an approach based on how long the chalazion has been present, its size, its effect on your eye and your preferences.
Warm Compresses and Lid Hygiene
Warm compresses, gentle lid massage and regular lid cleaning are the right first step for most new or small chalazia:
- Many chalazia settle without a procedure.
- Conservative treatment is appropriate when the lump is still improving.
- Ongoing lid care helps reduce the risk of new chalazia.
These measures have limits. In a randomised trial of 149 patients, four to six weeks of hot compresses, with or without antibiotic or steroid drops, completely resolved around one in five lesions. Chalazia present for more than two months were less likely to resolve without a procedure.¹
Incision and Curettage
Incision and curettage, a small incision followed by removal of the contents, is the surgical treatment for a chalazion that has not settled:
- Local anaesthetic numbs the eyelid.
- The eyelid is gently turned out to access its inner surface.
- The chalazion is cleared through a small incision.
- There is usually no visible scar and no need for stitches.
In another randomised study, incision and curettage resolved 87% of chalazia at three weeks, compared with 46% after hot compresses.²
Does Every Chalazion Need Removing?
No. Many resolve with conservative care, and your consultant will tell you if further warm compresses are worth trying.
We suggest treatment when a chalazion has persisted despite several weeks of regular compresses, is large or growing, affects vision, keeps returning, or is causing enough concern that you do not wish to wait longer.
What to Expect
Before Treatment
Your consultation includes an examination of the lump, eyelid margin and eye. Your consultant confirms whether it is a typical chalazion and explains the options, including whether waiting is reasonable.
If removal is appropriate, we discuss the procedure, expected recovery, benefits and risks. Treatment can often take place at the same appointment.
During Treatment
Chalazion removal is performed under local anaesthetic as a day-case procedure:
- Preparation: An anaesthetic drop numbs the eye, followed by a small injection to numb the eyelid. The injection stings briefly; afterwards, most people feel pressure only.
- Treatment: Your consultant gently turns out the eyelid and clears the chalazion through a small incision on its inner surface.
- Completion: The procedure itself usually takes around 20 minutes. An eye pad may be applied for a few hours to help limit bruising.
You return home the same day. Please do not plan to drive yourself home.
After Treatment
- Use chloramphenicol eye ointment twice daily for five days.
- Follow the written aftercare instructions provided.
- Most people return to normal activities within a day or two.
- Bruising usually fades over one to two weeks.
- Any remaining firmness softens over the following weeks.
No routine follow-up appointment is included. Contact our team if you have concerns during recovery; we will advise you and arrange a review if clinically needed.
Recovery and Results
- First few days: Most people resume normal activities within a day or two.
- First two weeks: Bruising gradually fades as the eyelid heals.
- Following weeks: Any remaining firmness softens, and the result becomes clearer.
Up to one in ten chalazia need a second procedure because the first has not fully cleared the lump.
For self-funding patients, the second procedure on the same chalazion is carried out at no additional charge under the Blue Fin Vision® Advantage, at any of our locations. There is no treatment, hospital or return-to-theatre fee, up to a limit of two procedures from one fee.
For insured patients, a second procedure is claimed through the insurer, subject to the policy. A new chalazion in a different place is a new episode of care and is charged separately.

A Surgeon’s Perspective
“A chalazion is a small problem, but it sits on your face and sometimes on your vision, so it rarely feels small to the person who has one. My job is to make sure it really is a chalazion, to tell you honestly whether it is worth waiting a little longer, and if it is not, to clear it properly with as little fuss as possible.”
