PresbyMAX vs SMILE

< 1 min read

PresbyMAX laser profiles address both distance and near vision, while SMILE mainly targets distance correction. They use different technologies, so the choice depends on age, presbyopia, and lifestyle.

Technology and indications

PresbyMAX is delivered via excimer laser, often with LASIK or surface techniques, and creates multifocal corneas. SMILE uses a femtosecond laser to remove an internal tissue disc through a tiny incision.

  • SMILE is currently used for distance correction rather than multifocal profiles
  • PresbyMAX suits presbyopic patients wanting reading-glasses independence
  • SMILE is ideal for younger adults without significant near problems
  • Corneal thickness, shape, and tear film are assessed for both options

Visual experience and trade-offs

PresbyMAX may compromise some distance crispness for near freedom. SMILE offers simple optics but continues to rely on readers as presbyopia develops.

  • PresbyMAX can cause halos or reduced contrast, especially at night
  • SMILE tends to have fewer visual side effects but limited near benefit
  • Contact lens trials of blended or monovision vision can preview PresbyMAX
  • Your clinician will help match the approach to your priorities and age
