A dropped nucleus occurs when lens fragments fall into the vitreous cavity after posterior capsule rupture. This requires pars plana vitrectomy performed by a vitreoretinal surgeon.
Understandably, this sounds serious.
However, modern vitreoretinal surgery is highly effective.
Most dropped nucleus cases are managed within a few days. At times surgery is delayed briefly, especially if corneal oedema obscures the view. Allowing the cornea to clear improves surgical safety and does not worsen visual prognosis ³.
Studies evaluating outcomes after retained lens fragments show that final visual acuity is often good when vitrectomy is performed in a timely manner ¹ ³. Complications such as retinal detachment or persistent macular oedema can occur, but these are uncommon with modern techniques ¹.
Recovery is typically slower than standard cataract surgery.
Expect:
- Blurred vision for several weeks
- More intense anti-inflammatory drops
- Follow-up with both cataract and retinal teams
Most patients stabilise around 4 weeks after vitrectomy ².
Dropped nucleus changes the recovery timeline, but not necessarily the final vision.
With appropriate management, visual outcomes remain strong ¹ ³.
