Dry Eyes After LASIK: Why Post-Operative Blurring Is Usually Temporary

This page is for patients who have had LASIK (laser in situ keratomileusis) and are experiencing dry eye symptoms, fluctuating vision, light sensitivity, grittiness, and want to understand what is happening, how long it lasts, and when to seek review.

Why LASIK Causes Dry Eye

LASIK severs corneal nerves in the flap interface.³ ⁵ These nerves provide the feedback signal that drives the blink reflex and stimulates lacrimal gland secretion.⁴ When disrupted, blink rate falls, tear production decreases, and the tear film becomes unstable. Variable vision, light sensitivity, and grittiness follow.

This is the most common post-LASIK complaint, and the one most commonly misinterpreted.¹ ² The laser correction is fixed and stable from day one. What fluctuates is the tear film sitting above it. Most patients who struggle in these weeks assume the procedure has not worked. It almost never means that.

The Timeline of Post-LASIK Dry Eye

  • Day 1 to Week 4: symptoms most prominent. Preservative-free lubricating drops are the primary management, used frequently, not sparingly.
  • Week 4 to 3 months: gradual improvement as corneal nerve regeneration occurs. Vision stabilises as the tear film becomes more consistent. This patient’s experience, solid vision at one month, is the expected trajectory.
  • 3 to 6 months: corneal sensation typically restored. Dry eye symptoms resolve in the vast majority of patients by six months.⁵

Who This Is Not For

This page is not for patients with significant pre-existing dry eye who were advised LASIK would worsen their condition. For those patients, ICL (implantable Collamer lens), which does not touch the corneal surface, may be considered as an alternative. If symptoms are severe or worsening, seek formal review rather than continuing to self-manage.
