Can blurred vision after cataract surgery be caused by something other than PCO?

Yes. Blurred vision after cataract surgery has multiple potential causes beyond posterior capsule opacification, and accurate differential diagnosis is essential before proceeding with YAG capsulotomy. Performing laser treatment without confirming PCO as the primary cause risks missing treatable pathology and failing to address the actual source of visual symptoms. ²

Retinal pathology is a critical consideration. Conditions such as age-related macular degeneration, epiretinal membrane, macular hole, diabetic maculopathy, or retinal vein occlusion can cause central visual blur. When PCO is present, it may obscure the clinical view of the retina, making detection of these conditions more difficult. ² Optical coherence tomography (OCT) before YAG capsulotomy allows identification of macular pathology even when the clinical fundal view is limited.

Cystoid macular oedema (CME), swelling at the macula, can occur weeks to months after cataract surgery and presents with blurred central vision similar to PCO. ¹ CME requires anti-inflammatory treatment, not laser capsulotomy. OCT imaging readily differentiates CME from PCO.

Residual refractive error is another common cause. If the implanted intraocular lens power does not fully correct the patient’s vision, spectacle refinement may be required. Dry eye disease, a frequent post-operative issue, causes fluctuating blur, surface discomfort, and visual disturbance that improves temporarily with blinking. ³ Corneal pathology, progression of pre-existing glaucoma, and amblyopia can also contribute to post-operative visual dissatisfaction.

A comprehensive clinical assessment including slit lamp examination, refraction, intraocular pressure measurement, and macular imaging ensures the correct diagnosis. Attributing visual symptoms to PCO without systematic exclusion of alternative causes is poor clinical practice.
