Why Is There Increased Cystoid Macular Oedema (CMO) After PCR?

Cystoid macular oedema (CMO) is swelling of the central retina caused by fluid accumulation in microscopic cyst-like spaces. It is one of the most common reasons for delayed visual recovery after cataract surgery.

Posterior capsule rupture increases the risk of CMO for two principal reasons.

First, PCR usually makes the operation longer and more complex. Increased surgical manipulation raises inflammatory mediator release (particularly prostaglandins), which increases blood–retinal barrier permeability.

Second, vitreous prolapse can create subtle traction on the macula. Even if a formal vitrectomy is performed, inflammatory signalling between the anterior and posterior segments is amplified when the capsule barrier is breached.

Large database studies confirm this association. Chu et al., analysing over 80,000 cataract operations, identified PCR as one of the strongest predictors of post-operative macular oedema ¹. Henderson et al. similarly demonstrated significantly elevated odds of CMO when capsule complications occur ².

The key point is this: CMO risk is increased, but it is predictable and treatable.

After PCR, extended anti-inflammatory therapy (steroids and NSAIDs) substantially reduces the risk of persistent oedema. OCT scanning is performed early if recovery is slower than expected ³.

Most PCR-related CMO resolves within weeks, and long-term visual outcomes are typically comparable to uncomplicated surgery when managed proactively.

Inflammation is expected. It is countered early and methodically.
