The corneal epithelium is biologically active and responds dynamically to stromal reshaping.
After laser eye surgery, the epithelium compensates for changes in stromal curvature. In myopic ablations, epithelial thickening tends to occur peripherally. In hyperopic treatments, central epithelial thickening may reduce the intended effect, contributing to regression ¹ ².
Modern high-resolution epithelial OCT mapping has demonstrated that epithelial remodelling can partially mask or modify the true stromal ablation profile ³. This explains why early outcomes may shift over weeks to months.
Hyperopic laser treatments are particularly prone to epithelial compensation and long-term regression ². For this reason, we are selective in offering primary hyperopic laser surgery.
Importantly, hyperopic residual refractive error after lens surgery behaves differently. Because the cornea has not been steepened by laser ablation, epithelial compensatory mechanisms do not produce the same regression pattern ¹.
Understanding epithelial behaviour allows us to:
- Time enhancement appropriately
- Avoid over-treating transient shifts
- Improve long-term refractive accuracy
Laser accuracy is mechanical. Corneal healing is biological.
Enhancement planning must respect both.
