Why Is Residual Hyperopia After Lens Surgery Different?

Residual hyperopia following cataract surgery or refractive lens exchange behaves differently from primary hyperopic laser regression.

After lens-based surgery, the refractive error originates from intraocular lens power calculation variability or effective lens position differences, not corneal steepening.

Because the cornea has not undergone hyperopic stromal ablation, epithelial compensatory remodelling is not stimulated in the same way seen after hyperopic LASIK¹.

Excimer laser enhancement in pseudophakic eyes has been shown to be safe, predictable and effective². Several studies examining LASIK enhancement following multifocal IOL implantation demonstrate excellent refractive precision without significant deterioration in optical quality² ³.

In many cases, corneal laser enhancement is safer than intraocular lens exchange, which carries risks including capsule rupture, zonular instability and cystoid macular oedema.

Posterior corneal astigmatism and effective lens position variability contribute to residual refractive surprises after cataract surgery³. Laser enhancement directly corrects this residual corneal plane error without re-entering the eye.

Unlike primary hyperopic laser treatment, stability after corneal refinement in pseudophakic patients is typically strong because corneal curvature has not been fundamentally altered in a regression-prone pattern.

Residual ametropia following lens surgery is therefore one of the most predictable indications for laser enhancement.
