When hyperopia is combined with astigmatism, treatment complexity increases. Larger peripheral optical zones and greater tissue redistribution amplify epithelial and stromal healing responses that contribute to regression.
Patients should expect slower visual stabilisation and a higher likelihood of enhancement procedures compared with simpler refractive errors. Outcomes can still be good, but they are more sensitive to individual healing responses and age-related changes.
Surgeons should discuss realistic timelines and the possibility that a staged approach or alternative treatment may offer greater long-term satisfaction.
