Ectopia lentis

Ectopia lentis is displacement or subluxation of the crystalline lens from its normal position. It can be hereditary or secondary to trauma or systemic disease.

Causes and signs

Degree and direction of displacement can vary.

  • Marfan syndrome, homocystinuria, and Weill-Marchesani as classic genetic associations
  • Lens edge visible on dilation, irregular astigmatism, and fluctuating refraction
  • Possible monocular diplopia or glare
  • Risk of lens-induced glaucoma if the lens blocks fluid flow

Management

Treatment is individualised based on vision and complications.

  • Glasses or contact lenses for mild subluxation
  • Surgical lens removal and possible lens fixation in more severe cases
  • Systemic evaluation for associated syndromes
  • Regular monitoring of intraocular pressure and retinal status
