What is canthoplasty?

Canthoplasty is surgery that reconstructs or reshapes the canthus, most often the lateral canthus, to improve eyelid support and appearance. It is more extensive than simple tightening (canthopexy).

Indications

Canthoplasty treats lower lid laxity, ectropion, entropion, or cosmetic concerns such as rounded outer corners. It may also be required after tumour excision or trauma.

  • Involves cutting and re‑fashioning the canthal tendon and lid margin
  • Can narrow or lengthen the palpebral fissure depending on design
  • Often combined with blepharoplasty or lid tightening procedures
  • Requires careful pre‑operative marking and symmetry assessment

Recovery

Temporary tightness, foreign‑body sensation, and corner asymmetry are common early on. These typically improve as tissues relax.

  • Lubricants protect the ocular surface while lid position settles
  • Follow‑up checks for over‑ or under‑correction, notching, or exposure
  • Minor adjustments can sometimes be made in the clinic by suture modification
  • Final cosmetic result may take several months to fully mature
