What is cantholysis?

Cantholysis is the surgical release or cutting of a canthal tendon, usually the lateral canthus, to relieve tension or allow greater eyelid mobility. It is often part of other eyelid procedures.

Uses

Lateral cantholysis can help correct tight lower lids, facilitate orbital exposure, or manage severe eyelid burns and cicatricial conditions. By dividing the tendon, the lid can be repositioned or lengthened.

  • Frequently followed by canthoplasty or other reconstructive steps
  • Must be carefully controlled to avoid excessive lid laxity or rounding
  • Can be performed in emergency settings to relieve orbital compartment syndrome
  • Knowledge of detailed anatomy is essential to prevent injury

Aftercare

Post‑operative swelling and bruising at the outer corner are expected. Subsequent reconstruction determines the final lid contour.

  • Eye lubrication protects against exposure while lids are temporarily loose
  • Follow‑up ensures stability and function of the repaired canthus
  • In emergency decompression, later cosmetic refinement is often planned
  • Patients should avoid manipulating the area until fully healed
