YAG capsulotomy typically takes between two and five minutes of laser time to complete the capsulotomy opening. ³ The total time spent in the laser suite, including patient positioning, contact lens application, focusing, laser delivery, and removal of the contact lens, is usually under ten minutes. The overall appointment duration, including pre-procedure preparation and immediate post-procedure checks, typically ranges from twenty to thirty minutes.
The laser procedure itself involves delivery of a series of individual laser pulses, typically 20 to 40 pulses depending on the thickness and characteristics of the posterior capsule opacification, to create a circular or cruciate opening in the central posterior capsule. ¹ Each pulse lasts only nanoseconds, but pulses are delivered sequentially with brief intervals to allow the clinician to reposition the focal point and assess progress.
The number of laser pulses required and the total energy used vary according to several factors: the density and type of posterior capsule opacification (fibrotic PCO typically requires more energy than regeneratory pearl-type PCO), the proximity of the posterior capsule to the intraocular lens (which influences energy settings to avoid lens pitting), and the surgeon’s technique and experience. ²
Patients spend most of their appointment time undergoing pre-operative preparation: pupil dilatation with mydriatic drops such as tropicamide 1% or phenylephrine 2.5%, which requires approximately 20 to 30 minutes to achieve adequate mydriasis; instillation of topical anaesthetic drops; and positioning at the laser. Post-operatively, intraocular pressure is often checked within 30 to 60 minutes to detect any acute pressure rise, and post-operative medications are prescribed. ² The brevity of the procedure itself is one of its advantages, as treatment is efficient and requires no recovery time in the traditional surgical sense.
