Is YAG capsulotomy suitable for mild posterior capsule opacification?

For mild PCO, YAG capsulotomy is usually not needed unless symptoms are disproportionately troublesome.

When mild PCO is best monitored

Slight capsule changes often cause little impact.

  • Good visual acuity and comfortable daily function
  • Minimal glare or haze that you can easily ignore
  • Other eye structures healthy and regularly checked
  • Understanding that YAG can be done later if symptoms progress
  • Preference to avoid even small risks while benefits are marginal

Exceptions where early YAG may be considered

Sometimes mild‑looking PCO causes significant problems.

  • Pilot, professional driver, or visually demanding occupation
  • Pronounced glare or contrast loss despite modest capsule thickening
  • Clear mismatch between chart vision and real‑world difficulties
  • Shared decision that a small opening could meaningfully help
  • Careful documentation of reasoning before proceeding
