Dense posterior capsule opacification is strongly suited to YAG capsulotomy, as visual benefit is usually significant.
Characteristics of dense PCO
The capsule acts almost like a new cataract.
- Marked reduction in vision with pronounced haze and glare
- Difficulties reading, driving, or recognising faces
- Posterior capsule visibly thick and fibrotic behind the lens
- Retina hard to inspect clearly until an opening is made
- High expectation of improved acuity and contrast after laser
Special considerations
Dense opacities may require a slightly larger or more careful opening.
- Possibility of more noticeable floaters immediately afterwards
- Need for closer pressure monitoring in the early period
- Re‑evaluation of retina and macula once the view improves
- Discussion of rare risks such as lens pitting or retinal tears
- Overall strong benefit-risk balance in most patients
