Floaters following YAG capsulotomy are typically temporary, settling over a period of days to several weeks in the majority of patients. ³ However, individual experience varies considerably: some patients report complete resolution within one to two weeks, while others notice persistent floaters that gradually become less bothersome as neuroadaptation occurs, meaning the brain learns to ignore them over time.
The natural history of post-YAG floaters involves gravitational settling of capsular debris inferiorly within the vitreous cavity, where it moves out of the central visual axis and becomes less noticeable. Smaller, lighter fragments may also be cleared by physiological processes involving macrophages and natural enzymatic breakdown of debris. ² The majority of patients find that floaters diminish significantly within four to six weeks post-procedure.
A minority of patients, estimated at fewer than 5%, experience persistent, visually disturbing floaters that remain functionally significant beyond six months. ¹ Studies examining this subgroup found that most had proliferative, pearl-type posterior capsule opacification at the time of YAG capsulotomy, which generates more substantial capsular debris than fibrotic PCO. ¹ In one case series of patients requiring vitrectomy for post-YAG floaters, the mean duration between YAG capsulotomy and vitrectomy was approximately 8.6 months, indicating that patients and clinicians typically allow a substantial observation period for spontaneous improvement before considering surgical intervention.
Persistent, bothersome floaters can be treated with YAG laser vitreolysis, using the laser to fragment large floaters, though this has variable success rates, or with pars plana vitrectomy, which provides definitive resolution but carries surgical risks. ² In a thorough clinical pathway, patients should be counselled pre-operatively that new floaters are common and usually temporary.
