Eye freckles

Eye “freckles” are small pigmented spots that can occur on the iris, conjunctiva, or retina. Many are benign, but some need monitoring.

Examples

Different structures show different types of pigmentation.

  • Iris freckles and naevi visible on the coloured part of the eye
  • Conjunctival ephelides or naevi on the eye surface
  • Choroidal naevi at the back of the eye seen during examination
  • Occasional association with conditions like ocular or oculodermal melanocytosis

Assessment and follow-up

Evaluation focuses on excluding malignancy.

  • Slit-lamp and fundus examination with photographs for comparison
  • Monitoring for growth, irregular borders, vascularity, or symptoms
  • Sun protection advice and review intervals tailored to risk
  • Prompt referral to specialists if features suggest early melanoma
