Central serous retinopathy CSR involves a blister of fluid under the central retina. It often affects young to middle-aged adults and is linked to stress and steroid use.
Symptoms and risk factors
CSR typically occurs in one eye at a time, sometimes alternating.
- Central blur, distortion, and objects appearing smaller or darker
- Metamorphopsia and a patch of missing detail when reading
- Association with corticosteroids, type A personality, and sleep disturbance
- OCT and angiography confirm serous detachment and leakage points
Course and treatment
Many episodes resolve spontaneously over weeks to months. Treatment is considered for chronic or recurrent cases.
- Observation and addressing triggers including tapering steroids if possible
- Photodynamic therapy or focal laser in selected chronic cases
- Newer oral treatments are being explored in specialist centres
- Regular monitoring to ensure no progression to permanent damage
