Floaters after YAG capsulotomy are small, dark spots, lines, or cobweb-like shapes that appear in the field of vision and move as the eye moves. They are caused by fragments of the posterior capsule that have been disrupted by the laser dispersing into the vitreous cavity, the clear, gel-like substance that fills the space between the lens and the retina. ³ These capsular fragments cast shadows on the retina, which the brain perceives as floaters.
Floaters following YAG capsulotomy are very common, experienced by the majority of patients to some degree. ³ The presence of post-YAG floaters does not indicate that the procedure has gone wrong; rather, they are an expected consequence of opening the posterior capsule. When the laser creates the capsulotomy opening, small pieces of the disrupted capsule, along with cellular debris and inflammatory material, are released into the vitreous cavity.
The characteristics of post-YAG floaters depend on the type of posterior capsule opacification treated. Proliferative, pearl-type PCO, characterised by clusters of swollen lens epithelial cells, tends to produce more noticeable floaters compared to fibrotic PCO. ¹ Studies of patients who required vitrectomy for persistent, visually disturbing floaters after YAG capsulotomy found that all had proliferative, pearl-type PCO at the time of laser treatment. ¹
Post-YAG floaters are typically most noticeable in the first few days to weeks following treatment, particularly when looking at bright, uniform backgrounds such as a clear sky or white wall. They are more apparent in bright lighting conditions. ² The floaters are generally harmless and do not indicate retinal damage or detachment, though patients should be counselled to distinguish between expected post-capsulotomy floaters and new symptoms suggestive of retinal complications such as sudden showers of floaters or flashing lights.
