However, the clinical pathway surrounding YAG capsulotomy requires more than technical competence with the laser. Accurate diagnosis of posterior capsule opacification, systematic exclusion of alternative causes of visual symptoms, recognition of risk factors for complications, appropriate patient selection, management of adverse events, and post-operative follow-up all require clinical judgement developed through experience. ³
In NHS settings, YAG capsulotomy is frequently performed by ophthalmology registrars under consultant supervision as part of their training. Studies confirm that resident-performed YAG capsulotomy achieves effective outcomes with a low complication profile when appropriate supervision and training structures are in place. ¹ Extended-role optometrists and specialist nurses with advanced training are increasingly delivering YAG laser services in both NHS and private settings, following competency-based training frameworks established by professional bodies. ²
In private practice, the procedure is more commonly consultant-led throughout the entire pathway. This typically reflects a greater degree of senior clinical oversight at every stage: initial assessment, diagnostic workup, laser procedure, and post-operative management. Patients are entitled to know who will perform their treatment and what level of clinical supervision is in place. Transparency about operator experience and the availability of senior clinical support is a reasonable expectation in any properly structured service. ³
