Dry Eye After Cataract Surgery: Why Vision Seems Worse Than It Is

Dry eye disease is one of the most frequently overlooked causes of suboptimal vision after cataract surgery. Because the symptoms, mild blur, fluctuating clarity, light sensitivity, overlap with those of residual refractive error or lens-related problems, patients and clinicians alike can be drawn toward optical explanations before the ocular surface has been adequately assessed.

Cataract surgery disrupts corneal nerves, alters the tear film and introduces topical medications that can compromise the ocular surface for weeks after the procedure. ² Studies suggest that 30 to 50 percent of early postoperative blur may be attributable to tear film instability rather than any optical or structural factor. ¹ ³

The tear film forms the first refractive surface of the eye. When it is unstable, light entering the eye is scattered before it reaches the lens or retina, producing blur that can closely mimic the effect of a prescription error. Crucially, this blur fluctuates – it is often worse first thing in the morning or in dry environments, and temporarily improves after blinking.

Effective management of the ocular surface, including lubricating drops, anti-inflammatory therapy where indicated, and lifestyle modification, frequently restores visual clarity without any need for further surgical intervention.
