Why Does Astigmatism Sometimes Remain After Laser Eye Surgery?

Astigmatism correction is uniquely sensitive because it is axis dependent. Unlike spherical treatment, which simply alters curvature centrally, cylindrical correction must be applied at the exact intended meridian.

Residual astigmatism after laser surgery may result from:

  • Cyclotorsion (subtle rotation of the eye when lying down) ²
  • Inaccurate pre-operative axis measurement
  • Healing-related epithelial redistribution
  • Biomechanical asymmetry of the cornea

Even small axis deviations matter. A 5–10° rotation can significantly reduce the effective correction of higher cylinders ¹ ². The larger the pre-operative astigmatism, the more unforgiving small rotational differences become.

In addition, posterior corneal astigmatism may not always be perfectly predicted, particularly in lens-based procedures ³. This can lead to mild residual cylinder even after technically excellent surgery.

Enhancement for residual astigmatism is common and highly predictable, especially when performed using modern eye-tracking and iris-registration systems ¹. Topography-guided treatments may be particularly valuable in irregular cases.

Residual astigmatism is rarely a sign of surgical error. It is usually a reflection of the vector complexity of cylindrical correction.

Precision matters, and enhancement exists to restore perfect alignment when necessary.
