- Other eye structures healthy and regularly checked
- Understanding that YAG can be done later if symptoms progress
- Preference to avoid even small risks while benefits are marginal
Exceptions where early YAG may be considered
Sometimes mild‑looking PCO causes significant problems.
- Pilot, professional driver, or visually demanding occupation
- Pronounced glare or contrast loss despite modest capsule thickening
- Clear mismatch between chart vision and real‑world difficulties
- Shared decision that a small opening could meaningfully help
- Careful documentation of reasoning before proceeding
