Lens replacement is not the best option when other treatments can give similar or better vision with less risk, or when the lens itself is still healthy and flexible.

In younger people, or where the main issue lies in the cornea rather than the lens, alternatives such as laser or ICL may be safer and more preserving of natural function.
- Under 40 with clear lenses and good natural near focus
- Moderate myopia suitable for laser reshaping of the cornea
- Eyes where ICL can correct extreme prescriptions without removing the lens
- Mild symptoms well managed with updated glasses or contact lenses
- People who are very risk-averse and already see well with correction
Other reasons to choose a different path
Even in older adults, lens replacement may not be ideal if eye disease or personality factors mean the visual trade-offs are unlikely to feel worthwhile.
- Significant macular disease or glaucoma limiting potential visual gain
- Severe dry eye or corneal problems where surgery may worsen discomfort
- Perfectionist expectations that no real-world procedure can meet
