Melkersson-rosenthal syndrome (MRS)

Melkersson-Rosenthal syndrome is a rare condition characterised by recurrent facial swelling, facial nerve palsy, and a fissured tongue. Eyelid and orbital tissues may be involved.

Ocular and facial features

The classic triad may not be present in every patient.

  • Recurrent painless swelling of the lips or eyelids
  • Episodes of facial nerve weakness causing lagophthalmos
  • Furrowed or fissured tongue glossitis
  • Granulomatous inflammation on biopsy

Management

Treatment is often symptomatic and tailored.

  • Corticosteroids or other immunomodulators for acute flares
  • Lid surgery or gold weights if exposure threatens the cornea
  • Speech and dietary advice for tongue involvement
  • Multidisciplinary care with neurology and dermatology
