What is Amsler grid testing?

Amsler grid testing is a simple method used to detect and monitor distortions or gaps in central vision, particularly from macular disease. It involves viewing a grid of straight lines with a central fixation point.

How it is Performed

The grid is held at normal reading distance, and one eye is tested at a time while the other is covered. The person focuses on the central dot and reports any areas where lines look wavy, blurred, or missing.

  • Can be performed in the clinic and at home for self-monitoring
  • Requires good near correction and adequate lighting
  • Both standard black-on-white and inverted versions are available
  • Findings may be drawn on the grid for documentation

Clinical Role

Amsler testing helps detect early changes in conditions like macular degeneration, macular oedema, or epiretinal membrane. New or worsening distortion prompts further assessment.

  • Useful for ongoing monitoring between clinic visits
  • Not a substitute for full retinal examination or imaging
  • Instructions should emphasise testing each eye separately
  • Interpreted alongside visual acuity and structural tests such as OCT
