Is ICL surgery suitable if you have glaucoma?

ICL surgery is generally approached very cautiously in glaucoma and is often not the first choice.

Concerns in glaucomatous eyes

ICLs can affect pressure dynamics.

  • Additional intraocular lens may narrow the angle and raise pressure
  • Risk of vault‑related pressure spikes or pigment dispersion
  • Need for ongoing, precise pressure monitoring after surgery
  • Glaucoma damage may already limit visual potential
  • Some glaucoma medications or prior surgeries complicate ICL placement

When it might be considered

Only highly selected cases are suitable.

  • Very early, well‑controlled glaucoma with open angles and stable fields
  • Compelling refractive need that cannot be met by other means
  • Multidisciplinary assessment with a glaucoma specialist
  • Detailed consent about uncertain long‑term pressure effects
  • Alternative refractive options usually discussed first
