Cataract surgery can be suitable with an epiretinal membrane, but visual improvement may be limited by macular distortion.
When cataract surgery alone may be enough
Some membranes are mild and stable.
- Mild epiretinal membrane with relatively preserved macular structure
- Blur or glare mainly explained by cataract on examination
- Desire to improve focus and light transmission before considering membrane surgery
- Regular OCT monitoring to track any membrane progression
- Use of monofocal lenses to maximise clarity and contrast
When combined or staged surgery is considered
More marked membranes may need vitrectomy.
- Significant distortion, wavy lines, or reduced best‑corrected vision
- Decision with a retinal surgeon about combined cataract-vitrectomy
- Explanation that even with surgery, some distortion can remain
- Awareness of slightly higher risk of postoperative macular oedema
- Individualised plan for timing and sequence of procedures
