Posterior capsular opacification

Posterior capsular opacification PCO is a common cause of cloudy vision months or years after cataract surgery. It occurs when the thin membrane behind the implant becomes hazy.

Symptoms and cause

PCO is sometimes called a “secondary cataract”, although the original lens has already been removed.

  • Gradual blur, glare, and halo around lights similar to the original cataract
  • Caused by leftover lens cells thickening and wrinkling the capsule
  • Does not usually cause pain or redness
  • Detected easily during a slit-lamp examination

Treatment

A quick laser procedure can restore clarity in most cases. Vision often improves rapidly.

  • YAG laser capsulotomy creates a small opening in the hazy capsule
  • Usually done in clinic with drops and no need for stitches
  • Risks are low but include temporary pressure rise and floaters
  • Most patients notice clearer, brighter vision within days
